The following information is needed for a patient license:

  • Patient’s full name
  • Residence and mailing address
  • Date of birth
  • Telephone number and email address
  • Physician information
  • Patient signature dated within 30 days of the application date

The following unexpired, valid documentation is required for the application:

  • Proof of Oklahoma residency (digital, color copy of one of the following):
    • Oklahoma driver’s license (front and back)
    • Oklahoma identification card (front and back)
    • Oklahoma voter identification card
    • A utility bill for the calendar month preceding the date of application, excluding cellular telephone and internet bills
    • A residential property deed to property in the State of Oklahoma
    • A current rental agreement for residential property located in the State of Oklahoma
  • Proof of identity (digital, color copy of one of the following):
    • Oklahoma driver’s license (front and back)
    • Oklahoma identification card (front and back)
    • U.S. passport or other photo identification issued by the U.S government
    • Tribal identification card approved for identification purposes by the Oklahoma Department of Public Safety:
      • United States Bureau of Indian Affairs identification card or a Oklahoma tribal photo identification cards: Muscogee (Creek) Nation of Oklahoma, Kiowa, Comanche, Apache of Oklahoma, Absentee Shawnee, Cheyenne-Arapaho, Choctaw, Delaware, Caddo Nation of Oklahoma issued on or after January 8, 2008.
  • A clear, color, full-face digital photograph
  • Adult Patient Physician Recommendation Form dated within 30 days of application submission
  • If applicable, the patient’s Medicaid (SoonerCare) or Medicare insurance card or enrollment documentation

In order to sufficiently complete the Minor’s application process, the following information is required about the minor patient:

  • Full name

  • Telephone number and email address (if applicable)
  • Information about two recommending physicians

The following information is required about the parent or legal guardian:

  • Full name
  • Residence and mailing address
  • Date of birth
  • Telephone number and email address
  • Signature dated within 30 days of the application date

  •  Proof of identity (digital, color copy of one of the following):
    • Oklahoma driver’s license (front and back)
    • Oklahoma identification card (front and back)
    • U.S. passport or other photo identification issued by the U.S government
    • For minors who do not have the items above, a certified copy of the minor’s birth certificate (please note that the seal/watermark will need to be visible when uploading the document)
    • Tribal identification card approved for identification purposes by the Oklahoma Department of Public Safety:
      • United States Bureau of Indian Affairs identification card or a Oklahoma tribal photo identification cards: Muscogee (Creek) Nation of Oklahoma, Kiowa, Comanche, Apache of Oklahoma, Absentee Shawnee, Cheyenne-Arapaho, Choctaw, Delaware, Caddo Nation of Oklahoma issued on or after Jan. 9 2008.

  • First Physician Recommendation Form
  • Second Physician Recommendation Form dated within 30 days after the First Physician Recommendation Form and dated within 30 days before the application submission date

  • One other government-issued (state or federal) form of identification
  • No discrepancies between the DOB & Legal Name listed on the two forms of identification

Of course! As long as the following requirements are met, Happy Hemp invites you to come check us out:

  • The medical card presented is issued by the State of Oklahoma and is not expired.
  • A valid form of ID is presented along with the OMMA Patient Card

Happy Hemp is committed to compliance, and our Point of Sale system stays update withe the Oklahoma Law to ensure that Happy Hemp does not exceed the daily sale limit per patient.

The back of your OMMA Patient Card goes into greater detail on the State Regulations on personal possession limits. 

Yes and no. All State and Federal tax exemptions are recognized by the State of Oklahoma; however, anyone purchasing cannabis products within the State of Oklahoma is obligated to pay a 7% Marijuana Excise Tax to the State of Oklahoma at the point of sale.

Unless the person(s) wishing to shop with you for cannabis products has the proper licensing required and requested by the State of Oklahoma, no unlicensed person(s) may enter into any part of any cannabis retailer in which the unlicensed person(s) would be exposed to cannabis.
